New Service Request

Welcome to Clay Battelle Public Service District.  To apply for water service, please contact our office (304-292-4003). New service and change of service applications are accepted at our drive thru Mon-Thurs 9am-1pm.  You can print out the Service Application and bring the completed form and required fees to our office Mon-Thurs at the Drive Thur Window between the hours of 9am and 1pm - or put the completed form with payment and copy of ID in the 24 hour drop off slot.   We must have a valid copy of your driver's license or identification card.  Incomplete or unsigned applications will delay service.  Please call the office in advance for the amount of the required fees.

Application Policy:

Clay Battelle PSD (CBPSD) requires all applicants desiring water to make written application on the provided form.  There must be a new completed form for each new location (move from one place to another).  Completed applications must be brought or mailed to our office with photo identification that matches applicant and required fees.  Any and all past due amounts on file in an applicant's name must be paid in full prior to new service being established. No person will have access to account information unless specified on the application.  A copy of renter's agreement or lease may also be required if applicant is a renter.  All fees must be paid before service is provided (new tap applications require additional paperwork and information).

Any change in the identify of the customer of record at a premise will require a new application for water service.  CBPSD may, after reasonable notice, discontinue water until such new application has been made and accepted.  However, the former customer shall remain liable for water service furnished to the premise until he/she has given written notice in the form of a completed  Service Disconnection Request Form (available under forms on this website) to CBPSD office.  In the event the customer of record has died or become incapable of being responsible for said service, the individual's spouse may become the customer of record without being required to pay new deposit(s).  However, they may be required to complete and sign an updated service application and provide a valid driver's license or identification card, if this information was not provided when the original application for service was completed.

All applicants must designate on the service application whether they are a tenant or owner of the premise to be served.  If the applicant is a tenant, he/she must include the name and address of the owner of the premise to be serviced on the service application.  As stated, a copy of the renter/lease aggrement that list the applicant and other adults may be required prior to water service.

No application for new tap(s) are not approved until reviewed by staff and, if necessary,  board members.

Applications may be denied until an applicant has complied with applicable rules of the Public Service Commission of WV or if the applicant's installation of piping equipment is regarded as hazardous or such character that satisfactory service cannot be provided.
